I certify that all information on this application and any other material provided by me is true and complete. I agree to preserve our clients’ confidentiality, protect survivor safety, maintain the confidentiality of all ATASK site locations, and adhere to all ATASK policies and procedures.
I further certify that I have disclosed any criminal convictions or pending legal matters. I understand that such information will be reviewed on a case-by-case basis and does not automatically disqualify me from volunteering.
For direct service volunteers, I agree to complete the 45-hour Domestic Violence training and undergo a Massachusetts Criminal Offender Record Information (CORI) check. I understand that ATASK may also, at its discretion and in accordance with applicable law, require a CORI check for additional volunteer roles, particularly those involving access to confidential information, access to ATASK locations, or public-facing roles representing the organization.
I understand that providing false or incomplete information may result in dismissal from the volunteer program.
